New care partnerships to be launched

Councils and NHS trusts will create a series of partnerships to help people with learning disabilities stay out of hospital and live in the community.

The new transforming care partnerships will cover the whole of England and be made up of clinical commissioning groups, NHS England commissioners and local authorities.
