Sheffield City Council has been criticised for failing to protect a 95-year-old woman who died after an accident in her home. Local government ombudsman Anne Seex said that despite complaints that the home help working for the care agency contracted to look after the woman had failed to report that she had fallen and broken eight ribs, the city council failed to take action. The woman died later after contracting pneumonia.
