Essex CC's adult social care team has helped slash the number of delayed discharges for care patients from local hospitals by more than 90%.
Figures for the 2011/12 period show the county, working with its health partners, oversaw an impressive 98% reduction in the number of people delayed while waiting for a residential care placement, and a 92% reduction in delays due to late assessment.
