Somerset CC has been found to have breached the Data Protection Act, after a teenager's personal details were sent to the wrong family.
An investigation by the Information Commissioner's Office (ICO) said the incident happened in February, when a council employee, who was working on two similar cases, sent a social service assessment about a teenager to the wrong family.
