'Missed opportunities' to safeguard neglected children

Four youngsters were left to live in ‘filthy and squalid’ conditions despite ‘numerous referrals’ to children’s services, a serious case review has found.

The children suffered from ‘significant and chronic neglect’ over many years, with their mother leaving them dirty and hungry while the eldest sexually abused his three younger siblings a number of times between 2010 and 2016.
