Social prescription is reducing demand in Wealden

Much of the conversation between the NHS and local government is around adult social care, the pressure on acute hospitals and delayed transfers of care. Given the very real issues around funding and integration that is a very important conversation. But it is not the only one.

District councils are playing their part in tackling adult social care and increasingly seeking interventions upstream – here is one example from Wealden.
